About The Advanced Care at Home Coalition

The Advanced Care at Home Coalition (the ACH Coalition) brings together stakeholders to create a pathway to coverage for advanced care at home services. In December 2022, ACH Coalition advocacy efforts resulted in the inclusion of a provision in the 2023 Consolidated Appropriations Act (CAA) that extends the Acute Hospital Care at Home waiver until December 31, 2024.

After this legislative victory, the ACH Coalition is focused on the development of a permanent pathway for advanced care at home programs that is accessible, safe, high quality, equitable, and innovative. The ACH Coalition is also working to ensure that CMS and lawmakers understand its benefits and its role in defining the future of the U.S. health care system.

Background on Advanced Care at Home

During the COVID-19 pandemic, federal and state governments issued waiver flexibilities that allowed hospitals to provide advanced level services to patients at home under certain circumstances. The waiver flexibilities built on the success of previous hospital at home models that have been tested over decades, showing that advanced care at home can be a safe, effective way to provide care to patients. Although these flexibilities were originally tied to the public health emergency (PHE), the ACH Coalition succeeded in extending the waiver through December 31, 2024. However, more work is required to ensure the permanency of hospital at home programs.

The ACH Coalition’s Objectives

By bringing together industry stakeholders, the ACH Coalition is amplifying and advancing a federal strategy to create a permanent pathway for continued access to advanced care at home. We are working to build momentum in the following ways:
